Concreting a sloway

Hello i am wanting to concrete my sloping driveway. The driveway at the moment is grass, stones etc.
I know I have to get rid of all the debris etc., but when it comes to doing the drive I have a few questions.
Will I have to put a found either side and at the bottom and build a block wall for the concrete to sit into.
It is sloping down towards the house.
Any help would be greatly appreciated thanks.
 
Normally you would dig out approx 300mm then fit edgings to contain the concrete/hardcore.
 
If it's slopes towards the house is there somewhere for the rain to drain away??
