When we did our drive we used paving expert site (Google it) and ended up with a fantastic job. Possibly over kill, but the materials were cheap and the labour free.
For foot traffic dig out any soft spots then I'd guess from memory 75mm min hardcore compacted and maybe 50mm concrete, but reading that site will give you far more understanding than I could ever help.
Good luck!

The pukka job would be 100mm sub base and 100mm concrete, You could get away without sub base as long as the ground is undisturbed for many years but you must use at least 75mm of concrete. 50mm of concrete will crack for sure. 75mm would be ok in smaller slabs but pouring that as one lump you'd be better with 100mm.

At my own house i have used 75mm of sub base and 85mm ish of concrete.

To make it 100mm thick you would need 1.3cubic metres. At that volume it will be cheaper to buy readymix and get it delivered. Mixing up that quantity will take 2 bulk bags of gravel (1520kg) and 1 bulk bag of sand (820kg) plus 17 bags of cement.
