You can always isolate by some means or other.

Insulated screwdrivers are made for precautionary purposes. Not so you can work live - Although some HV jobs are done live - but they take a lot more precautions than your ordinary sparks.
 
the DNOs join underground cables live all the time, I don't belive any significant number of them get killed doing it. Its just a matter of planning out how to do the job safely and then executing that plan carefully.

It would be interesting to compare the number of sparkies who get killed each year while working live to the number of sparkies that die each year while driving to thier jobs. Sadly i've got no idea where you'd find information on it.

EAW 1989 regulations prohibit live work except in special circumstances.
