Electrical Installation Certificates

Afternoon,

I'm in need of an Electrical Installation Certificate template of some description which can be filled out to forfill the requirements of BS7671, My cert. pads were stolen on site so i have noway of recording test results untill the NICEIC sends me some new ones.

Would be very grateful if someone could give me a template of a certificate or,**************************

Just to make it a little bit more interesting, you need to click on the link for the 1992 forms in order to get the 2004 ones
I have emailed the webmaster over on the iet regarding this.
