The wiring terminals are 0, 1, 2, 3, 4, 5 top to bottom.

0 is common return from the door pushes.
1 is the front door push - chime which can be changed by rotating the dials.
2 is the back door push - chime is default "Come to the Cookhouse Door"
3 is power from transformer
4 is power from transformer

I am not sure if 3 or 4 is +ve or -ve

I had marked my cables before removing unit while decorating, then realised I had not noted whether numbered top to bottom. Searching on the net there were no wiring diagrams for this unit, so having successfully re-wired it, I have made a note which may help someone else wire one of these, or maybe me next time I take it down.

They are neither. Transformers are AC devices so have no +ve or -ve.
