Funny clutch acting up...

Any suggestions as to the following problem;

10year old Escort, 100k miles

The clutch biting point is very high, a few cm's from the top.
In first and second gear car makes a 'whining' noise like a turbo sound
If i release the clutch too quickly (read: normal speed) the car jerks off as it engages and pulls away.

At some point the clutch has been replaced but could it be going again?
 
Check adjustment,could be over adjusted (if it's a cable).

If adjustment is correct or it's a hydraulic system then you probably need a new clutch.
