Gap between upvc front door frame and floor

Hi, Im currently decorating my hallway and found the concrete floor next to the upvc front door frame with a crack in it most of the way along the frame, on a closer inspection found the concrete to be loose so I removed the loose material.

With this loose material removed I can now push a long screwdriver down and move the outside driveblock.

I would appreciate some advice to remedy this ?
