in the uk the regulations require no gap greater than 99mm
not sure about the usa

1. In brief,

the IRC requires a max. ht. for handrail of 34" to 38" from the tread nosing.

a min. width of stairs 31 3/4" with one rail.

1 1/2" min between wall and rail.

2. You will require a Newel post at the bottom tread and fixing brackets (pin them to studs) along the wall. Fixing the newel is the most difficult bit for a DIY'er.

3. A single rail must run up the LH side only. Dont try to jog the rail out or in at the wall, run it straight.

4. There are kits of newel, rail & spindles available on the web and at HD or Lowes. They come with instructions. Figure approx. two spindles per tread.

4. As big-all above, you must space the spindles with a max. 4" gap between them.

The width of the stairs is the key. Good luck.
