How do I open this old type of manhole cover?

Found this in the garden covered by undergrowth. Only words I can find on it is 'PATENT' and nothing is moving. Do the rectangular blocks move first or something?

Probably just stuck with age but I think you should clean it up a bit and expose all of it so that people can see what you are dealing with.
