How to level this threshold?

Hey there

Due to the clusterf*ck that was the builders who built our extension (whole other story), I've been left which thus threshold that needs to be levelled with the rest of the floor.

The door is installed on the outer skin of block work, and there's then 100mm concrete and a 50mm insulation break between the outer and inner skin of block work, with a plastic dpm layer on top.

The height difference to the chipboard floor varies between 10mm and 25mm, so not sure on best approach.

Are there any self-levelling products that will fill that depth and also adhere to the dpm?

Plan is to put 9mm flooring ply across the entire floor including this area, so it doesn't need to be perfect, just level with the rest of the floor...

A significant height difference can be lost if it extends over a large floor area. I would try underlay to make up the difference
 
I would look to see what self levelling you can use.
