Kitchen tap head replacement HELP.

Hi, this is first post so apologies in advance if this is in wrong place.

I have a tap in my kitchen and the head is corroded (as per image) it is leaking out of the tip that slots over the spout. I’ve looked online via different searches and cannot find if this part is replaceable or even what it’s called.

Ideally don’t want to have to replace the whole tap as the house is pretty old and no easy way of turning of water to the tap.

sorry I should have mentioned the corrosion as led to a split, which is spraying water out.
 
Well the end part is called the Aerator but I doubt you will find a bent one the same pattern.
This place does a range I noticed they also do Universal spouts if you scroll down, maybe worth a try.
