Knocking a door an old external wall

Hi All

Been a while since I last posted but have thrown myself into getting the next stage of my house done. I did have a builder come and fit (with my help) some beams up the attic for a strong floor but alas he never finished the job or completed the two knock throughs to the extension I had paid him for (to be honest I'm not sure I would have trusted him to do it right.

So I could do with some advice, there are lots of people showing how to put a lintel into a single brick wall but that's all that is shown doesn't answer the questions I have.

So the jobs are
1 Knock through breeze block cavity wall on first floor to gain access to the extension bedroom.
2 Knock through same wall in the attic to connect for access to electrics and aerial.

I have acro props and a strong boy, need to get mortar, lintels and whatever I need to put under the lintel, Door frame and door.

So the questions
1) This is a old external end wall, What grade/type of lintel should I be using?
2) What should I be putting underneath the lintel, I have read that I should have a couple of courses of Engineering bricks?
3) Chances are the lintel isn't going to line up nicely with the mortar line. Do I just chop the lintel sized hole where I need it then fill above with brick to make up potential gap to next mortar line or cut blocks as needed above lintel and fill gap with mortar?
4) What do you do about finishing / the cavity? The door frame will be mounted inline with one course of blocks and won't be wide enough to cover the gap.
5) I'm assuming that the done thing is to chop down to just below floor board level and then extend the floor board across the threshold of the door?

Mostly I am concerned with getting the Door way right, The attic access worries me slight as it is close the ridge but I'm sure with careful cutting this will be fine and the hole will get lined with MDF and a basic door made.

Any help or advice on the questions I have listed or anything else I should know / consider that I might not know would be gratefully received. I had really wanted to get a builder to do this job but I just can't find one I feel I can trust and am sure I am capable of doing this work.

At a later date I need to know through from the living room to the garage on the ground floor with a Fire door. This isn't till later on but if there is something different that I should do when doing this door, please say.
