I had a very similar problem a few years back... the cutout fuse carrier had lost its mechanical 'springyness', and was starting to char away.
The Electricity Distributor replaced and tested the meter TWICE during their attempts to repair, and on neither occasion noticed the fuse carrier was damaged, despite removing and refitting it to replace the meter.
Eventually the power just failed one night, so I called them out again. This time it was spotted, and then a 'live works' crew were sent to replace the entire cutout as it had been heat damaged too.
A few days later, my fridge, freezer and washer-dryer all curiously failed.... nothing to do with the Electrical Distributors negligence though you understand....

Bell transformer output wires
Actual I meant whatever that thing is on the top row, it says 12v on it and has some thin wires with no sheath coming out of the bottom of it and out of the top of the unit. Maybe it's not actually powering a door bell but it's of that ilk.
Considering the recently retrofitted British General RCBOs and the slightly painful angle the busbar seems to be taking to accommodate them, I'd be looking at the connections out of the bottom of the main switch. How is the busbar for the bottom row of breakers connected to the output of the main switch? It's not entirely clear in the pic. Myself, I'd want to take everything out of those terminals for full examination and then replace as appropriate, possibly with new busbar. I'm not suggesting you should attempt this yourself though - you should get someone with the necessary qualifications and experience in to do that task.
Another possibility is that a contact has failed in the main switch itself.
Overall, I'd say you need to have the whole thing looked at quite carefully, possibly with a view to replacement. It doesn't look like it was the best of installations in the first place, and it's obviously been hacked about quite a bit in the years since.
