We have taken out rear window and door to give opening into new extension but notice that there is a concrete lintel on inner leaf of cavity brickwork but no lintel on outer leaf just the bricks, the house was built in 1950's anf window and door frame was only 40mm thick

do we need to put in any new lintel or rsj ?

Yes. Presumably the outer skin was supported by the frame, so you will need a replacement for that. Steel beam would probably be simplest - size depending on span.
 
Steel was in short supply after the war well into the 1950's, so the lack of a lintel is common.

Yes, you should insert a lintel into the front skin, and if you have removed a flag window frame then you will most probably have to Acrow/Strongboy prop the brickwork.
