loft reinforcincig rafters

How are reinforcing rafters installed for beefing up the old rafters when doing loft conversions,i cannot see how a full length rafter can be installed next to the old rafter from ridge to wall plate when the roof is still covered with felt and tiles,do chippys cut them just short of the wall plate?.

Generally the additional rafter is being put in in order to take the weight of a dormer or suchlike and this means that a hole is being cut in the felt anyway so that the whole thing goes in from the outside. It is possible to do it (and I have) by only cutting the felt so far up and feeding the new rafter upwards but it's an akward job for no real benifit.
