Our living room has a terrible layout, and is giving us bad necks from the ridiculous angle we have to watch tv at.

We have a real chimney and flue for the gas fire, but ideally would like to create a media unit. Shelves on left, inset TV and fire.

Building it etc isn't the issue, plenty of experience there. The thing I'm not sure about is the fire.

Obviously we would have to bring the wall put to inset the TV and run the wires, but what happens with the fire?

Can we bring the fire forwards with some sort of gas fire extension, or could we put in a long fire with some sort of extension to bring it out?

I guess the fire would have to come out 2-3ft

Forget about putting the TV over the fire. it will ruin it quickly.
You will also get neck ache looking up there.

Angle the TV unit into the room more and re arrange the sofa's

Can't do that, sofas and TV are fixed due to doors.

We've got recliners so but either way, a long inset fire is likely needed over the one we actually have at the moment. This type of thing (grey walled one) https://www.flames.co.uk/blog/media-walls.html
