Mixer tap screw handle removal

I need to replace the cartridge on a mixer tap but need to unscrew the small metal cap at the top of the handle to get to the grub screw. The cap won't move under hand pressure and there is very little metal to grip so does anyone have a trick to removing these without breaking the ceramic?

Based on another set of taps that I have (I think they are both Sottini), the cap unscrews. On the other set, when I replaced the cartridge a few years ago I applied grease to the thread and I was able (today) to unscrew the cap. It's a really poor design meaning (unless there is a trick I don't know) that it's necessary to destroy the ceramic once the cap has been in place for a while in order to remove the cap.
