Patio doors wobble at top

Hi all we have had patio doors around 10year now they are still great condition but when close or open or push at top it seems to be very loose like moving away from the wall.The outside you can see a lintel over them seems to be mental would they screw into this when fitted just not sure what to do is it just foam in there any help be great
