Patio On This Concrete

Hi I want to build a patio on this concrete dimensions 360cm x 360cm I have the length, but I don't have the width, I have to create another 100cm to reach the width, so I was going to create a form for the width, concrete it, and sort of join it to the existing concrete, also as you can see the existing concrete is not too level or great so would need to repair and level off, or should I just rip out the concrete and start over, total novice, any advice appreciated.
