Normally you must test it unless there is an excetional reason not to.

If you do not test IR, it must be noted as not tested in the 'extent of installation tested' box
 
What if there's fixed appliances etc would you just IR btween Lives and Earth, with both lives together

That is the acceptable way if you are in any doubt about connected/sensitive equipment

This should be entered in the "limitations" section, not the extent.
 
If u not sure (per above) IR test between live and neutral together and earth.

Dont go live to earth and neutral to earth. That may still scrog some IT equipment that has mains input filters.
