Problem with my coal/wood fire place

Can someone offer some advice please, we have lived in our current home for 3 years. We have had the fire place swept every year and often asked the sweeps adivce on an ongoing problem. The fire plate is coming away from the wall, 1st he said it was because we had no ballast in the surround and it was not sealed when you put your hand up the chimney to feel around the top of the fire. I filled the voids in the surround with gravel and cemented the top as suggested. This year it has come away form the wall by at least 30mm and pushed the surround off also. Any idea what this is and how I can fix it, Once I get home from work tonight I can take some photos if I have not explained myself correctly.
