I want install a 'Hole in the Wall' type fire, but obviously this needs to be higher than the hearth. Are bricks and concrete the answer?

A hearth can be raised by a variety of non-combustible materials, blocks and mortar being two of them.
It depends how you want the finished article to look and whether you will be cladding the masonry with tiles or plaster or marble etc.
