A couple of farm jacks might help, along with a couple of plates to provide a stable surface?
You don't have much space
Possibly jacking up in a series of lifts, alternating front/back left / right and adding bits of smaller timber to get height in excess of the replacement sleeper, then add the new sleeper and reverse the action

Easy enough to lift up each end with 4x2 over a brick as lever and slide a section of plastic decking under ( looks like neighbour has).
