I'm not convinced it comes under RIDDOR. The only mentions of electricity (barring offshore stuff) in the RIDDOR regulations relate to plant machinery coming into contact with overhead power lines and electrical fires/explosions.

I think it would be more appropriate to report it under the EaWR to the HSE. I reported it to my boss, it's in his court now.
 
In writing I hope? It seems with health and safety verbal is not good enough must be in writing even if that writing is on back of a fag packet or as a SMS but must be in writing.
 
Cool! But they should have used a stronger knot to stop the cable falling. What did it supply and how did it end up being switched on again?

I was disconnecting woodworking machines yesterday in a rather large (awesome) old workshop that had closed and was being cleared, 2 of the machines had been wired without any earth connection, and had probably been in use for at least the last 40 years. Both the machines had red (!) earth wires which were just left in small coils inside the junction boxes. Lots of lovely cast iron switchgear and fabric/rubber singles there, imperial conduit everywhere.
 
They used to supply some milling machine or other, not too sure on the specifics. The machine broke down, I gather. And then was disconnected and removed. None of the supplies in the 3P board were labelled, so if it was ever switched off, it was switched back on again by mistake. Other supplies had been removed and put in chocboxes/joint boxes and were still energised, for some reason, this guy was just looped off the cable tray and tied onto this steel.
 
The supply feed should have been disconnected at the outputs of the feeding breaker itself at the origin.
 
I've only ever come across 2 MK 3Ph boards, they can't have been popular in this part of the country.
 
I can't remember seeing an MK 3 phase board! Do they even still make them?

But you were both correct. MEM. The lovely boards with those awful metal U clips for the screws to go into, that are invariably half missing
