Can anyone explain why in some installations and circumstances is the armour terminated at 1 end, both ends and what is the general ruling?

Amongst other reasons
Because with two core cable, there would be no earth without using the armour ,or running an independant earth

As far as I know

Unless for a specific reason, the cable should be terminated into a gland at both ends regardless of number of cores, even if the armour is not used as the only means of earthing.

Ideally also with an earth lead of suitable size, lugged to the banjo with a suitable brass nut and bolt, and connected to the earth terminal of the switch or whatever.
 
522-06-03:

Except where installed in conduit or duct, which provides equivalent protection against mechanical damage, a cable buried in the ground shall incorporate an earthed armour or metal sheath or both., suitable for use as a protective conductor, or be of insulated concentric construction.......
