vent through roof

I need to install an extractor fan through the ceiling which would mean it coming out onto a flat roof. I'm not sure what the best way dealing with the vent on the flat roof side is though. Any pointers much appreciated.

I did exactly this last year. What you need is one of these. Screwfix only do the 45 degree version, but if you go to a decent building supplier you should be able to find the straight version as well.
