The ceiling of the bathroom is outside of restricted zones - what about the ceiling of a wetroom? If this is restricted what IP rating of down lights do I need?

Apply the same rules as a bathroom, zone-wise. If the ceiling will get sprayed, fit IP66 SELV fittings and protect the lot via an RCD.

Personally I would fit IP rated fittings wether it will be in the zone or not. Don't forget there will be steam in the room which could present a hazard with some fittings, and help to corrode some parts (like lampholder terminals) of some fittings.
