What is this Tap Valve?

I would appreciate any help in identifying this tap valve. The internal thread is 3/4” and it is ¼ turn on/off. My main problem is that it had a square head to the shaft rather than the normal spline.

perhaps it was a lever-action tap (surgeon's tap) with a ceramic cartridge.

Why do you ask?

Did you see the tap it came out of?

My guess would be that is an adaptor pressed on to the spline otherwise it would never have passed through the stuffing box on the tap body.
