My neighbour has moved to a fairly new house. Was looking under the sink as you do. Noticed a kind of pressure gauge on the incoming main. With a pipe going to the waste as well as to the water supply to the house. Any ideas? Never seen anything like this before

Probably a pressure reducing valve.

Do they have an unvented hot water cylinder.

If so the expansion relief valve may be plumbed into the waste pipe - this is a bodge as it should go to the outside.

A photo would be usefull.
 
They do have a unvented tank with a pressure vessel where the cold feed should be.
