Need some help. This wall in the kitchen is wet through from the bottom up to about half way up. I have taken the plaster off as it was just falling off as its soaked. The wall is in our kitchen and I think its the original wall which now has an extension off it. Any help appreciated

I don’t be checking that radiator out for leaks first
How is the roof of the extension connected to this wall ? Pics of exterior more useful than interior.
