wood floor to solid floor

I will be taking up a wooden/joist floor in lounge and hall to concrete and render instead.
Question.
1) is 150mm concrete 90mm celotex and 150mm screed correct?
2) The DPM should the lounge be one sheet rolled up at door to hall then a separate sheet at hall where lounge door meets hall?
3) should the concrete/screed at door of lounge be done in one fill then hall in separate fill so a join in effect is at the door to two areas?
