Worcester 24i pressure fault

Looking for some advice, i have had a problem with My 24i pressure. i am constantly having to top up the pressure in the system, once topped up the pressure will either continue to rise or over time it will drop of completely. i changed the PRV but the fault is still there, i am getting water still dripping from the overflow.
