Doorstep selling "right to cancel" notice

Has anyone found a form to use when selling in a customers home? Obviously needs to include a "right to cancel" notice.

Seco, I do appreciate that, and thanks, but I was earlier pointed to a form that had been posted onn the cc last year.

This forum, by the way, is so much more useful and helpfiul than "the other one"
