Polypipe fitting failed after 1.5/2years

Hi does anyone know why a polypipe fitting may fail on a central heating system 1.5/2 years since install?

More information needed On plastic tube? which type of polypipe fitting they make several. Fitting fail completely or drip?
Picture would be good.
 
I haven't taken the floor up yet. It started with a damp patch on the ceiling over a couple of weeks and now it has started dripping though the ceiling.

Assuming that it's been correctly assembled fully pushed home insert in place and pipe not damaged/scratched.
If fitting has failed you need to check out polypipe guarantee.
 
Thanks I'm hoping for a nail. Have had that before where the nail has held pressure then rusted and started to leak.
 
It wasn't tested to 18 bar I know poly say to do this I do on new builds but on domestic central heating install It hasnt been.

All they will do is replace the fitting.

Actually not! some years ago we had a 15mm grey poly elbow fail after around 3 months.
Open vented system elbow failed over night large amount of decoration damage to ground floor plus carpets.
When you looked at fitting in effect 3 of the grab ring teeth were missing.
All costs (including ours) paid by polpipes insurers.

So was likely it came out of the factory like that? Did they check to see if had been tested to 18bar. Apparently they can tell by how far the grab ring has bit in to the pipe.
