Accidents due to new cable colours

Are there any records which show the number of accidents or fatalities that have arisen as a direct result of the introduction of the new Blue/Brown colour scheme for domestic & industrial wiring?

I was fitting some new lights the other day, and the innards of some the junction boxes looked quite colourful.
 
That is why it is IMPERATIVE that all installations using mixed colours need to be marked up as such, in several places, if necessary.

I make it policy to stick one in every house I visit: if it is not needed, at least it has raised awareness in the worker's minds.

That's a good idea. Whilst there is a sticker on my consumer unit, I will put some stickers on the lids of the junction boxes where the two colour codes join. Future generations can't go wrong, then.

To date I have not heard of any accidents as a result of a mix up with the new colours.

You can get special round warning stickers for joint boxes, but unfortunatly they don't seem to sell them at TLC.
 
The red/black to brown/blue scenario has been around for years, for this reason I am led to believe it only a requirement for a notice to be posted at the origin of supply for a single phase installation.
In a 3phase installation it is a little bit more complex, where red-brown, yellow-black, blue-grey, black-blue is more confusing. This is why the interface is required to be marked up as L1, L2, L3, N.
 
Do round ones exist to go on junction boxes? Whilst it might be a requirement to post a notice at the origin of supply only, this does somewhat typify the short sightedness of those who decide on such things. Surely, additional warnings at the point where the colours change can only lead to greater safety and lower risk of mistakes in the future?

The only place I have seen them is at ECS electrical wholesalers in Leeds.

This probably doesnt help you much though.

Best bet is probably to try your local wholesaler and see if the stock them, or can get them.
