Bosch PSB 18 Li2 Hammer Drill Fault????

I bought the above hammer drill from Homebase and i have used it only once and the time that i did the drill bit seemed to work itself loose from the chuck. I was drilling into masonary using the hammer action and when i pull the drill bit out out of the wall it was left behind. Has anyone had this problem, or knows how to rectify it??? Also when you tighten the drill bit in the chuck it tightens but you then can tighten it further and then it clicks..........should this happen?

My bosch drill will click 6 or 7 times before it is tight.
