Below is a pic of my current setup.

The toilet is old (perhaps 30yrs) and the waste (1) seems to be cast iron.

To the right of the toilet there is a cast iron soil stack (which has been boxed in)

I would like to get a modern looking closed coupled toilet in here... However what i wanted to know is whether it will be possible to do a straight swap, without any modification to the soil stack?

The waste (1) sits approx 100-150mm away from the wall

Close coupled will go in the space, but your issue is going to be the soil connection. A ? shape pan connector will get you back closer to the wall, but you may still find you have a gap left between the cistern and the wall. Usual practice then is to fix a batten to the wall to make up the gap, then tile it to form a shelf.
