It looks remarkably clean, so if it's been like that for long nothing nasty is getting in
The covers for those cisterns are notoriously fickle and not fitting properly and then falling off, you might find it on the floor. TBH it's a toilet so it doesn't really matter what gets in there. Sometime I use a bit of duct tape to hold them on, never really lasts though.
