A fault from what to what? If there is no 'earth' anywhere to be seen, it presumably can't be to that?

Are you Perhaps talking about a situation in which the first fault is from one side of the supply to 'something touchable' and the second fault is from the other side of the supply to something which is simultaneously touchable?

It would either have to have a lot of slack or be elastic - since, unlike the situation with buildings on earth the required cable length would vary appreciably over time :-)
