Does anyone know of a solution for connecting exposed hot and cold water pipes to a mixer or thermostatic shower?
 
You can get exposed versions of showers that cater for this, with drop down/upcoming feeds.

I’m having serious issues finding a mixer/thermostatic shower with dual bottom feed connections
 
These are all rear feed shower bars, I understand how those elbow connectors will allow exposed pipes to connect...however this solution creates a problem of the riser bar being offset from the wall...
 
The elbows which allow for the bottom feed to the shower bar offset the shower bar from the wall, therefore the outlet feeding the riser bar will also be offset meaning the riser bar will require an extended fitting to the wall...

Do you mean the shower riser rail? The vast majority are a shower hose connection and won't be affected by the distance from the wall. Did you have a specific type of shower valve in mind?

Might be best if you post a photo of existing shower so we get what you mean.
