external door threshold design?

What would be a good design for an external door threshold?
Do regs req. that it has no steps, access for disabled and some kind of weathering bar?
What is the relationship of the DPC to Ground level and finished floor level.
 
I cant believe there is not reply to this since 2005

I am new to this site, have I missed something
