We had one the other week where a local electrician was convinced he had found a crossed polarity.
What he had found was a property with no earth and a live to earth fault on a lighting circuit. Our jointer fitted a TN-S earth and left him trying to find the fault!

Yeah, some faults can be right pigs.....

I came across an installation with reverse polarity as an apprentice. The main switch had been tailed up back to front. It took me ages to work out what was going on. I'd been left there by my tradesman to do a bit, and could not fathom out what was going on.
