A decade ago my uncle was knocked down and killed by 17yr old lad and got away with it! So yes people die, relatives die
The problem with dumbing it all down is that eventually people won't have the experience to recognise danger.

They should ban mobile phones from the workplace if they want a safer working environment.

IOSH recommends it as do the HSE - most major contractors enforce a ban and have done for at least the last 5 years AFAIK. Same goes for Walkmans, iPlayers, DVD players, personal radios, etc (although that doesn't stop anyone form using them on Saturdays....... ). Maybe not a bad thing as it at least gives me a way to stop the bloody apprentice fiddling with his phone every 5 minutes.....
