How can I re do my bathroom

Hi guys I'm re doing my bathroom and have taken off all the tiles.

1. I've found out they tiled over the existing tiles ( I didn't even think this was possible ) should I remove those as well

2. Some of the plaster is coming off should I re plaster everything.

3. Can I change the lay out ( it's so small )

4. Any other tips on how I can make it look super nice

I would treat the mould. I don’t think you need to re-plaster if you are tiling just get it reasonably level and make up with tile adhesive.
 
All I can suggest is that you bash an alcove into the wall and set in a slimline basin (something longer and narrow), and if you're tiling it yourself it might be worth getting the walls skimmed - much easier for a novice tiler.
