Insulation upstand at door threshold

I think I might have made a small mistake! When I screeded the floor, I put upstands all the way around including the door threshold - but in the cold light of day I'm thinking that it's really not gonna be suitable for tiling right up to the door threshold. I.e., the tile overhanging the insulation.

Should I dig it out and fill the gap? Or is there a trick I'm missing?

You fitted the upstand in the correct place. You would usually fit the door over the insulation. Just tile over it, make sure you have plenty of adhesive between the tile and insulation. It's only there for cold bridging.
