Well, the outlet is quite near her patio. It could be extraction from their toilet. It's also only a metre or so above the ground, as the neighbours house is a bit lower.
I can't believe someone could be so stupid as to think that it's ok to do that sort of thing, without consultation, and presumably trespassing into her garden to fit the cover. Just like you wouldn't run your gutter outlets into someone's garden, or make your building overhang the boundary.
And as with trees, if your neighbours tree overhangs your garden, you're allowed to cut off the offending branches (as long as you give them back).

So it's not just the principle, it's also a complete lack of common sense on the part of her neighbour. Takes all sorts....
