I need one for a job.
One job.

You'd think it would be easy... high-torque low-speed drill, down to 150rpm, 240V, impact not required, half-inch chuck.
Seems to be a unicorn.

Easier in 110V, but I'm in line for a new hip, can't be lugging one of those transformers around.

Any recommendations please?

Yes, I have a Titan... but not much oomph at low speed.

A variable speed is no use if you need low speed AND torque, as the speed reduces, so does the torque. You need a drill, which is either low speed, or includes a gearbox to reduce the speed to what you need.

SDS are quite slow, could you run one without the hammer?

worth pointing out
dewalt do a 1296w 18v drill 126 nm so quite torquie in the dcd999 now not cheap but gives an idea whats about

If you get a drill with a load of torque you've still got to hang on to it.

You may yet be better strappng the tree rather than this somewhat unusual approach. Or even dare one suggestt that if the state of the tree is worrying, then it may be time to remove it for safety.

Have you tried high speed with low pressure?
Thinking out loud here - drilling green wood, you're wanting to cut rather than tear, so I wouldn't think low speed and high torque would be an advantage.
Also thinking that the higher the torque you're applying, the more likely you are to injure yourself or snap the drill inside the tree...
