Monobloc tap leak

Hi

I have a monobloc tap fitted in my basin.

This morning I saw some water dripping from underneath the sink. It seemed to originate from underneath the base of the tap.

I also realised that the tap is somewhat loose. Additionally, I dismantled the body and saw that the ceramic block is slightly cracked around the edges.

I would be grateful if someone could offer some advice on how to remove and reseat the tap and if replacing the ceramic block might resolve the issue.
