So the question is how much (as a percentage of the total cost) is the final payment? Obviously don’t pay it until you’ve had sone kind of resolution
 
He still thinks that he has done nothing wrong :(

That’s called Builder’s Brass Neck. You see it a lot when mistakes like this are made !

He’s clearly a crap builder, unfortunately the best you are likely to get now is a discount.
 
Get him to knock it down and restart.

I would start talking business with him: either he rebuilds the extension square or sue him.

There is a written contract and it is in his name. though he has asked my friend to make payments into a number of accounts including a limited company account.
 
Dodgy builders get paid in other's accounts.
Limited companies are just a get out of trouble legal loophole.
As the contract is in his name, I would start investigating whether he has any assets in his name.
Go on the land registry website and purchase the records for his address (£7)
If he owns his house, even if mortgaged, you can play the court card.
If not, he'll walk scot free unfortunately.
Once one of my best bricklayer made a mistake measuring one side of the brick, so the extension was shorter on one side by 4 inches.
Fortunately I found out before dpc, 6 courses.
We took it down and restarted eventhough 4 inches would've been not immediately noticeable.
Mistake happen, but a good builder would have more than 2 eyes on the job.
A foot is a massive mistake, I'm surprised they didn't notice earlier.
