It is a medieval Swiss army knife primarily used for branding or blinding?

A ring sizing mandrel?

I really don't think this is a medical instrument. It was mixed in with screwdrivers and wrenches, nuts and bolts, etc.

Looks like it's designed to get hot, could be for opening a tube out and trimming?

I agree looks like a tool for de-burring metal pipe work, conduit etc..
Or for anal fissure surgery
